Community Support Program
Transitional Living Services has intensive case management programs designed to support adults with severe and persistent mental illness. The Community Support Program offers an array of treatment options and services such as housing, employment, education, social skills, and help with family relationships and alcohol or drug abuse. The Community Support Program services are long-term but transitional, focusing on a consumer's passage toward independence.
Among the services available are:
- Advocacy
- Assessment
- Psychiatry
- Nursing
- Case planning
- Supportive therapy
- Medication management
- Life skill training
- Pre-vocational training
- Money management
- Social network management
Also available are services offering links to other community support networks, such as legal assistance, housing assistance, treatment for alcohol or drug addiction, and medical treatment.
All clinical treatment is provided by TLS staff members, who work with the consumer as a team to assist in achieving therapeutic treatment and life goals. The team comprises the consumer, a case manager, a psychiatrist, a psychiatric nurse, and other individuals the consumer chooses to involve.
Each consumer has a primary case manager responsible for coordinating treatment with other team members. Consumers meet regularly - usually weekly - with an assigned case manager to discuss treatment issues. When necessary, a case manager will accompany a consumer to a community appointment.
To be eligible for participation in the Community Support Program, a consumer must:
- Be 18 or older.
- Have a severe mental illness such as schizophrenia, bi-polar disorder or major depression.
- Have experienced difficulties in the last six months with housing, employment, medication management, legal issues, money management, or a worsening of symptoms that required emergency intervention or hospitalization.
