List of TLS achievements for 2005

HOUSING

  • TLS Milwaukee Housing Program expanded by 14 beds for a total of 164 in the Milwaukee community with the addition of the 8-apartment Washington Avenue property in West Allis.
  • TLS Racine Housing Program expanded by 14 beds for a total of 27 in the Racine community with the addition of the 6 apartments Douglas Avenue property.

CLC – Community Learning Center-Milwaukee

  • 64% increase in referrals from 2004
  • 10% increase in number of individuals served from 2004 for a total of 124 with no increase in budget or staff
  • Self-improvement/symptom management groups gained great popularity and had as many as 20 consumers participating in one session
  • Strong resurgence of night outings at least twice per month with consumers choosing the activities
  • Consumer artwork created during Donna Lexa art classes donated by consumers for TLS fundraising
  • Weekly addition of an arts and crafts class
  • Weekly computer labs offered focusing on using the Internet
  • Community Learning Center was re-modeled. Consumers now have more room to move around the Community Learning Center and the expansion allows for concurrent programming to occur

FAMILY CARE PROGRAM

A case management program for adults 60 years and older with a mental health issue contracted with the Milwaukee County Department on Aging.

  • Family Care Program is now at capacity providing 95 consumers with access to services such as supportive home care, meals, day programs, and durable medical equipment.
  • Family Care Program services are in high demand and currently has a waiting list of consumers referred by the Milwaukee County Behavioral Health Unit who would like to be transferred into the program. Because of the efforts of the TLS team, many of our Family Care consumers are able to stay in their homes and "age in place."

TLSU

A health and wellness program for TLS consumers.

  • TLS University (TLSU) continues with a total of 16 students in Milwaukee and Racine with the curriculum centered on wellness, addressing subjects ranging from nutrition to psycho-education and symptom management. The cohort of students who started in April 2004 will finish their curriculum early this summer and graduate. Students have and will demonstrate an increased knowledge of health-related issues and many have increased their activity level and improved their nutrition as a result.
