Targeted Case Management
The Targeted Case Management program offers an array of coordination and referral services geared toward helping TLS consumers become and stay independent. The program provides assessment, case planning, procurement of services and ongoing monitoring and service coordination. A case manager is assigned to each consumer to assist in mapping out an individualized treatment plan.
Targeted Case Management services may include:
- Legal Assistance
- Assistance with Daily Living Skills
- Employment, Training and Education
- Medical Services
- Benefits Advocacy
- AODA Services and Support
- Transportation
- Outpatient treatment and/or Psychotherapy
Targeted Case Management services are long- or short-term but transitional, focusing on the consumer's passage toward independence. The decision to
discontinue services is made jointly by the consumer and the consumer's service providers.
To be eligible for participation in the Targeted Case Management services, a consumer must:
- Be 18 or older.
- Have a severe mental illness such as schizophrenia, bi-polar disorder or major depression.
- Have experienced difficulties in the last six months with housing, employment, medication management, legal issues, money management, or a worsening of symptoms that required emergency intervention or hospitalization.
